Business Assistance Form

This form is used by the Economic Opportunity and Development Division (EODD) to learn about businesses in Cambridge. You can use this form if you have a Cambridge business now or if you are planning to have one.


Someone from EODD will contact you by phone or email after you have completed the form. We may need to ask follow up questions or clarify your answers. When we contact you, we will also share information that can help you right away and in the future. Some of the help we offer includes grants, free workshops, and tools for business planning. We can also connect you to city staff in other departments that issue permits and licenses.


Some questions on the form are not required right now but may be required later.


City staff from EODD can help you fill out this form over the phone or in person. We can also provide translation services over the phone.


The information you provide is confidential and is not shared with third parties.
